Skilled Nursing Services

Licensed Nursing Care at Home

Hands-on clinical care delivered in the home by Florida licensed Registered Nurses (RN) and Licensed Practical Nurses (LPN), following the plan of care written with your physician.

Skilled Nursing Care (RN & LPN)

Florida licensed nurses provide hands-on clinical care at home, from head-to-toe assessments to complex treatments ordered by your physician.

What this includes

Our Registered Nurses and Licensed Practical Nurses bring hospital-level clinical skill into the comfort of your own home. Every case begins with a full nursing assessment and a written plan of care developed with your physician, then continues with scheduled visits, hourly shifts, or around-the-clock nursing depending on what your condition requires. Nurses monitor vital signs, evaluate changes in condition, carry out physician orders, document each visit, and keep your doctor and family informed so nothing is missed between appointments.

Post-Hospital & Transitional Care

A nurse meets you at home after discharge to set up medications, review orders, and reduce the risk of readmission.

What this includes

The first days after a hospital or rehab discharge carry the highest risk of complications and readmission. Our transitional care nurses review your discharge instructions, reconcile every medication, set up equipment and supplies, teach you and your family the warning signs to watch for, and coordinate follow-up appointments. Visits are scheduled closely at first and then spaced out as you stabilize, so recovery continues safely at home instead of back in the hospital.

Wound Care & Post-Surgical Nursing

Skilled dressing changes, surgical site care, and treatment of pressure, diabetic, and slow-healing wounds.

What this includes

Our nurses manage surgical incisions, pressure injuries, diabetic ulcers, venous wounds, drains, and negative pressure wound therapy using sterile technique and physician-directed protocols. Each wound is measured, photographed where appropriate, and documented at every visit so healing progress is tracked objectively. We teach caregivers safe dressing technique, watch closely for infection, and report changes to your surgeon or physician immediately.

Medication Administration & Management

Nurses administer medications, reconcile prescriptions, fill pill organizers, and watch for side effects and interactions.

What this includes

Medication errors are one of the most common reasons people return to the hospital. Our nurses reconcile every prescription and over-the-counter product, set up weekly medication boxes, administer oral and injectable medications, and monitor closely for side effects, interactions, and missed doses. We work directly with your physician and pharmacy when a regimen needs to be simplified or adjusted, and we teach families how to manage medications confidently between visits.

IV Therapy, Infusion & Injections

IV antibiotics, hydration, injections, PICC and central line care delivered safely at home by trained nurses.

What this includes

Infusion therapy no longer requires a hospital stay. Our nurses administer IV antibiotics, hydration, and other physician-ordered infusions, care for PICC lines, midlines, and central venous catheters, perform flushes and dressing changes, and give scheduled injections including insulin, anticoagulants, and B12. Sterile technique, pump management, and infection prevention are documented at every visit, and any adverse reaction is escalated to your physician right away.

Chronic Disease Management

Ongoing nursing support for heart failure, COPD, diabetes, hypertension, stroke recovery, and kidney disease.

What this includes

Living with a chronic condition takes daily management, and small changes matter. Our nurses track weight, blood pressure, blood sugar, oxygen levels, and symptoms for conditions such as congestive heart failure, COPD, diabetes, hypertension, stroke, Parkinson's disease, and chronic kidney disease. We teach disease-specific self-management, reinforce diet and fluid restrictions, review medications, and alert your physician early — when a phone call can still prevent an emergency room visit.

Catheter, Ostomy & Feeding Tube Care

Skilled management of urinary catheters, colostomies, PEG and G-tubes, including family teaching.

What this includes

Our nurses insert, change, and troubleshoot indwelling and suprapubic urinary catheters, care for colostomy, ileostomy, and urostomy sites, and manage PEG, G-tube, and J-tube feedings including pump setup and flushes. We monitor for infection, skin breakdown, blockage, and dehydration, and we train family caregivers step by step until they feel confident handling routine care between nursing visits.

Diabetic Care & Blood Glucose Monitoring

Blood sugar monitoring, insulin administration and teaching, diabetic foot checks, and nutrition guidance.

What this includes

Diabetes management at home covers far more than a daily reading. Our nurses monitor blood glucose and A1C trends, administer or teach insulin injection and pen technique, review carbohydrate and meal planning with you, inspect feet and skin for early signs of ulcers or neuropathy, and coach you on recognizing and treating hypoglycemia. Findings are reported to your physician so insulin and oral medications can be adjusted safely.

Respiratory & Oxygen Therapy Support

Oxygen management, nebulizer and inhaler teaching, tracheostomy care, and breathing assessment.

What this includes

For clients living with COPD, asthma, pulmonary fibrosis, or long-term effects of respiratory illness, our nurses assess lung sounds and oxygen saturation, manage oxygen concentrators and portable tanks safely, teach correct nebulizer and inhaler technique, provide tracheostomy and suctioning care, and reinforce breathing and energy conservation techniques. We also review home oxygen safety with the whole household.

Pain Management & Palliative Support

Comfort-focused nursing for serious illness, with careful symptom control and family support.

What this includes

When comfort is the priority, our nurses assess pain and symptoms at every visit, administer and adjust physician-ordered pain medication, manage nausea, shortness of breath, restlessness, and constipation, and provide gentle personal care that protects dignity. We support families with education, emotional presence, and clear communication with the physician or hospice team so that decisions are never made alone.

Nursing Assessment, Care Plans & Care Coordination

A free in-home nursing assessment, a written plan of care, and ongoing coordination with your physician.

What this includes

Every client begins with an in-home assessment by a Registered Nurse covering medical history, medications, mobility, fall and safety risks, nutrition, cognition, and home environment. From that assessment we build a written plan of care with measurable goals, share it with your physician for approval, and update it as your condition changes. Our office coordinates with doctors, therapists, pharmacies, equipment suppliers, and family members so everyone is working from the same plan.

Therapy Services

Physical, Occupational & Speech Therapy

Licensed therapists help you rebuild strength, regain independence in daily activities, and restore speech and swallowing after illness, injury, or hospitalization.

Physical Therapy (PT)

Strength, balance, gait, and mobility training at home to restore movement and prevent falls.

What this includes

Licensed physical therapists evaluate strength, balance, gait, endurance, and pain, then build a home exercise program you can actually keep up with. Treatment covers post-surgical rehabilitation after joint replacement or fracture, stroke and neurological recovery, fall prevention and balance retraining, safe transfer and walker or cane training, and pain reduction techniques. Progress is measured at each visit and reported back to your physician.

Occupational Therapy (OT)

Retraining the daily activities that matter — dressing, bathing, cooking — plus home safety adaptation.

What this includes

Occupational therapists focus on the activities of daily living that make independence possible: dressing, bathing, grooming, cooking, and managing the home. They retrain fine motor skills, upper body strength, and coordination, recommend and teach adaptive equipment such as reachers, tub benches, and dressing aids, evaluate the home for hazards, and work on energy conservation and cognitive strategies after stroke or injury so that daily routines become manageable again.

Speech Therapy (Speech-Language Pathology)

Treatment for speech, language, cognition, and swallowing difficulty after stroke or illness.

What this includes

Licensed speech-language pathologists treat difficulty speaking, understanding, or being understood after a stroke, brain injury, or with progressive conditions such as Parkinson's disease and dementia. They also evaluate and treat dysphagia (swallowing difficulty), recommend safe food and liquid textures to reduce choking and aspiration risk, retrain memory and problem-solving skills, and coach families on communication techniques that reduce frustration at home.

Respiratory Therapy Support

Breathing treatments, airway clearance, and pulmonary education for chronic lung conditions.

What this includes

For clients managing chronic lung disease or recovering from pneumonia and respiratory illness, our clinicians reinforce breathing exercises, airway clearance, and pulmonary rehabilitation techniques, verify correct use of nebulizers, inhalers, and CPAP or BiPAP equipment, and monitor oxygen saturation and symptoms. Care is coordinated with your pulmonologist and home equipment provider.

Medical Social Services

Help navigating benefits, community resources, counseling, and long-term care planning.

What this includes

Illness affects far more than the body. Our medical social work support helps families understand benefits and coverage options, connect with community resources and support programs in Florida, plan for long-term care, and work through the emotional strain of caregiving. We assist with advance directives, caregiver burnout, and difficult family conversations, and we help arrange the additional services a client may need.

Personal Care & Homemaker Services

The Non-Medical Support We Have Always Provided

The compassionate day-to-day help our team has delivered for years, now working side by side with our nursing and therapy staff on one coordinated plan of care.

Home Health Aide & Personal Care

Bathing, grooming, dressing, toileting, and mobility assistance provided with dignity and respect.

What this includes

Certified Home Health Aides and CNAs assist with bathing, showering, grooming, oral care, dressing, toileting and incontinence care, transfers, and safe walking, always under the supervision of our nursing team. Aides follow the same written plan of care as the nurses, report any change in skin condition, appetite, or behavior, and give families the reassurance that someone attentive is there every day.

Companion Care & Disability Support

Companionship, supervision, and everyday assistance for seniors and adults with disabilities.

What this includes

Isolation takes a real toll on health. Our companions provide conversation, shared activities, reminders, supervision for clients with memory loss or fall risk, and support for adults living with disabilities. They accompany clients to appointments, social outings, and errands, and keep families updated on how their loved one is really doing day to day.

Meal Preparation & Nutrition Support

Nutritious meals prepared to your taste and to the diet your physician or nurse has ordered.

What this includes

Our caregivers plan, shop for, and prepare meals that follow physician- and nurse-directed diets, including diabetic, low-sodium, renal, heart-healthy, and modified-texture diets for clients with swallowing difficulty. We encourage hydration, monitor appetite and weight changes, report concerns to the nursing team, and make sure meals are something the client actually looks forward to.

Homemaking, Housekeeping & Errands

Light housekeeping, laundry, grocery shopping, errands, and transportation to appointments.

What this includes

A safe, clean home supports recovery. Our team handles light housekeeping, laundry and linens, dishes, tidying, grocery shopping, prescription pickup, and errands, and provides transportation and escort to medical appointments. Caregivers also keep walkways clear and report clutter, lighting, or bathroom hazards that could lead to a fall.

Respite, Live-In & 24-Hour Care

Hourly, overnight, live-in, and around-the-clock coverage so family caregivers can rest.

What this includes

Family caregivers need relief before exhaustion turns into a crisis. We provide scheduled respite by the hour, overnight coverage, live-in caregivers, and continuous 24-hour staffing with nursing oversight. Whether you need a few hours a week or full-time coverage while you travel or recover yourself, care can be scaled up or down without changing agencies.

Home Safety & Fall Prevention

In-home safety evaluation, fall risk reduction, and equipment recommendations.

What this includes

Falls are the leading cause of injury for older adults in Florida homes. Our nurses and therapists evaluate lighting, flooring, stairs, bathrooms, and furniture layout, recommend grab bars, shower chairs, raised toilet seats, and mobility equipment, and train the client and family in safe transfers and walking. Recommendations are written down and reviewed again as mobility changes.

Paying for Care

Let Us Help You Sort Out Coverage

Every family’s situation is different, and coverage depends on your diagnosis, your physician’s orders, and the plan you carry. Call our office and we will walk through the options that apply to you before care starts, so there are no surprises.

Bring these to your first call: your diagnosis and recent hospital or discharge paperwork, a current medication list, your physician’s name and phone number, and your insurance or long-term care policy details. A Registered Nurse will review them during the free in-home assessment.

Not sure which level of care you need?

That is exactly what the free in-home nursing assessment is for. A Registered Nurse visits, reviews the medical situation, and tells you plainly what is needed — nursing, therapy, personal care, or a combination.
